Please print the appropriate form or forms and mail (to Family Promise of Santa Clarita Valley 18565 Soledad Canyon Road, #133- Canyon Country, CA. 91351) or FAX ( to (661) 621-1792) or bring it to Box City on October 1st. Sorry, but no one will be admitted without a properly completed Registration/Release/Waiver of Liability. If you have any questions contact Terry.
